I am a 54 year old female approximately 75 pounds overweight. Had a spontaneous dvt in my left leg 16 years ago. I had knee surgery to repair a torn meniscus 1.5 yrs ago in my right leg, after that I had a small pulmonary embolism in my right lung. Now I am a "lifer" on Coumadin. Is it ok to have EVLA while on meds?
You are at high risk for a blood clot, and the EVLA can cause a blood clot. Your radiologist can help you make a risk assessment to see if the benefits outweigh the potential risk.

I have been doing EVL procedures for 9 years and I have not stopped blood thinners (warfarin, Xarelto, etc) for these procedures. Patients do very well.

Yes, it is safe to perform an ablation while fully anticoagulated. In my opinion, an Interventional Radiologist , is perfect for image guided endovenous ablations.

EVLT can be performed by various specialists trained to do it. Certainly, an interventional radiologist trained in closure procedures, would be capable of doing it. I routinely perform EVLT with patients on coumain without stopping the coumadin. If you are uncomfortable with you physician then you should get another opinion.

Too difficult to answer fully without seeing you, but ablation procedures can be done when on coumadin.
You should be certain your deep venous System is open however, if you are going to seal the surface veins.
